I realize that there is a risk of
injury inherent in all sports and
that the risk may be severe, including
but not limited to fractures, paralysis
or death. I hereby give the consent
for myself/my child to participate
in surfing activities that also involve
swimming and travel to and from the
beach. I authorize the contest director
and/or her agents to seek any emergency
medical care that may become necessary
for myself/my child in the course
of such athletic activities or travel
and understand that the cost of such
medical care will be at my sole expense.
I hereby knowingly and unconditionally
indemnify and release the Florida
Surfing Association, the Sisters of
the Sea, the City of Jacksonville Beach, the sponsors
of the event, the contest director
and/or her agents or anyone acting
on her behalf from any and all responsibility
or liability for any injury or damages,
whatsoever, incurred, suffered, or
caused arising from my/the above named
child's participation or presence
at this event. Release is also given
for the use of any photos or videos
to be used in future advertisements
and/or articles.
