I realize that there is a risk of injury inherent in all sports and that the risk may be severe, including but not limited to fractures, paralysis or death. I hereby give the consent for myself/my child to participate in surfing activities that also involve swimming and travel to and from the beach. I authorize the contest director and/or her agents to seek any emergency medical care that may become necessary for myself/my child in the course of such athletic activities or travel and understand that the cost of such medical care will be at my sole expense.
